Transitions Hospice Grief Support Groups

Transitions Hospice offers Grief Support Groups the second Thursday of each month at its Naperville office, 1551 Bond Street, #143, beginning August 13, 2019. The support groups begin at 6:30 p.m. The dates of the groups are: August 13, 2019; September 10, 2019; October 8, 2019; November 12, 2019 and December 10, 2019. Everyone grieves differently and it can be helpful to talk about the loss of your loved one and how your life has changed. Transitions Hospice hosts grief support groups for adults who have experienced the death of a loved one to explore and discuss their feelings in a supportive, comfortable and confidential environment, while meeting others who have experienced loss. SWHPN 2018 General Assembly

We are excited to announce that Transitions Hospice Executive Director, Sara Dado, LCSW, has been selected to be one of the speakers at the Social Work Hospice & Palliative Care Network (SWHPN) 2018 General Assembly which will take place at the Boston Park Plaza Hotel in downtown Boston from March 11-13. This will be Transitions’ 3rd year presenting at this annual conference. NASW – Indiana Chapter 2017 Annual Conference

Transitions Hospice of Indiana Executive Director Shannon Countryman RN, CHPN and Social Worker Catherine Ray, MSW, LCSW, ACHP-SW, CCTP, CT have been invited to speak at this year’s NASW Indiana Chapter Annual Conference (September 25th & 26th). This invitation gives the Transitions team the unique opportunity to educate community social workers on the important role of the hospice social worker within the interdisciplinary team setting.  This conference will allow others to see the progressive footprint and customized approach to care of Transitions Hospice. Both Shannon and Catherine bring years of valuable knowledge and experience to our hospice teams. Shannon has served in various clinical, operational and business development roles during her seven years in hospice. Educating others on hospice and high quality end of life care is her passion. Shannon received CHPN certification in 2013 and was recently elected to the Board of Directors for the Indiana Hospice and Palliative Care Organization. Shannon is a true advocate for hospice, a passionate leader and an asset to our growing Indiana team. Catherine Ray’s passion for long term care patients began twenty four years ago when she began her career as a Social Services Director in a skilled care facility. She has been caring for hospice patients for over ten years.  Catherine enjoys educating professionals, students, and the community about grief and loss, a topic not often  addressed in the healthcare arena. In 2016, Catherine received the Flanner & Buchanans Caregiver of the Year award. This is just a small glance into what makes Transitions one step ahead of the rest. The Illinois Health Care Association (IHCA) is having it’s 67th Annual Convention & Expo from September 11 to 14 in Peoria, IL! On Thursday, September 14, Transitions Hospice Executive Director Sara Dado, LCSW, will conduct a talk on “Did You Just Say the “H” Word? Overcoming Barriers in End of Life Care” from 8 to 9:30 am. About the Talk: The word “hospice” can be scary and intimidating to families, patients, and health care workers. How do we, as health care providers, respond when families ask us not to use the word “hospice” in discussion surrounding our plan care? This session will address common objections to hospice care and provide practical techniques to overcome these obstacles. Focus will be given to the historical background of hospice care as well as cultural and spiritual beliefs surrounding end-of-life care.
