Through this expansion, Transitions will be serving patients in Detroit, Lansing, East Lansing, Troy, Royal Oak, Livonia, Pontiac, Novi, Farmington Hills, Ypsilanti, Ann Arbor, and more. The addition grows Transitions’ presence beyond Illinois and Indiana to provide both hospice and palliative care services to a growing number of patients in the state of Michigan.
