Transitions Executive Director Sara Dado Elected to ILHPCO Board of Directors

Transitions Group would like to take a moment to congratulate and recognize Sara Dado, LCSW, Executive Director of Transitions Hospice on her newly appointed seat with the Illinois Hospice and Palliative Care Organization (ILHPCO). Transitions Home Medical Group Accepted Into National Pilot Program

Transitions Home Medical Group was recently chosen as one of the elite 30 health care organizations across the country to participate in the inaugural class of the CAPC Payment Accelerator Pilot Program. This year long program will enable Transitions Home Medical Group to expand its mission of bringing much needed medical and psycho-social support to more seriously and chronically ill patients across both Illinois and Indiana. Transitions Home Medical Group provides both primary and comprehensive palliative care services to those individuals who qualify through a home and community based service model.
