Transitions Hospice of Indiana Executive Director Shannon Countryman RN, CHPN and Social Worker Catherine Ray, MSW, LCSW, ACHP-SW, CCTP, CT have been invited to speak at this year’s NASW Indiana Chapter Annual Conference (September 25th & 26th). This invitation gives the Transitions team the unique opportunity to educate community social workers on the important role of the hospice social worker within the interdisciplinary team setting. This conference will allow others to see the progressive footprint and customized approach to care of Transitions Hospice.
Both Shannon and Catherine bring years of valuable knowledge and experience to our hospice teams. Shannon has served in various clinical, operational and business development roles during her seven years in hospice. Educating others on hospice and high quality end of life care is her passion. Shannon received CHPN certification in 2013 and was recently elected to the Board of Directors for the Indiana Hospice and Palliative Care Organization. Shannon is a true advocate for hospice, a passionate leader and an asset to our growing Indiana team.
Catherine Ray’s passion for long term care patients began twenty four years ago when she began her career as a Social Services Director in a skilled care facility. She has been caring for hospice patients for over ten years. Catherine enjoys educating professionals, students, and the community about grief and loss, a topic not often addressed in the healthcare arena. In 2016, Catherine received the Flanner & Buchanans Caregiver of the Year award.
