What is Palliative Care?
Palliative care is a team approach to treating patients with chronic and/or terminal illness in home based settings. Unlike hospice, palliative services are appropriate for any person that has a chronic and/or terminal illness, regardless of life expectancy, in which additional support is needed to manage symptoms. Palliative care services can be provided in any setting and for a variety of different diseases and symptoms.
At Transitions, our palliative team consists of palliative care trained physicians, advanced practice nurses and licensed clinical social workers who work in conjunction with primary care physicians. Our goal is a holistic treatment approach that addresses both medical and psychosocial needs, including symptom management, counseling and community service support.
Palliative care at Transitions is conducted through teams in the Transitions Home Medical Group (THMG). THMG offers a variety of community-based and inpatient services for those that qualify. Our patient and family-centered services focus on quality of life, along with the prevention and treatment of suffering. Palliative services include:

RevUp Remote Patient Monitoring and Health Coaching
RevUp is a software used by Transitions Home Medical Group that offers patients a personal Health Coach that will help them successfully achieve their health goals by managing and reporting on aspects of day-to-day health. This information is constantly monitored by THMG’s Symptom Management Team. providing them with live, up-to-date information on the patient’s health and current outlook. This allows us to visit with patients in a timely and efficient manner when they need us most, as well as help Transitions monitor our patients even when we are not present – furthering our promise of 24/7, 365 palliative and primary care.
