Community Health Accreditation (CHAP) is an independent, non-profit accrediting organization for both home and community-based health care organizations. CHAP is a nationally approved and has “deeming” authority granted by the Centers for Medicare and Medicaid Services (CMS). This means that CMS recognizes accreditation status from CHAP as meeting or exceeding Medicare standards.

At Transitions Hospice and Palliative Care, we are proud to say that we are CHAP accredited in both Illinois and Indiana. This means that our hospice and palliative programs meet the industry’s highest quality standards. CHAP accreditation demonstrates to our patients and families that we are providing services that exceed both quality and compliance standards.
